Henoch-Schönlein purpura

- Clinical features
- Young (<20 y/o), palpable purpura, abdominal pain, GI bleeding, arthropathy, fever, fatigue, ~IgA ↑ (50% p’ts), hematuria, proteinuria, macroscopic hematuria immediately after URI in children, NS (uncommon)
- GN is very similar to ~IgA nephropathy
- Skin: Leukocytoclastic vasculitis, dermal ~IgA deposits
- Kidney biopsy is indicated only for p’ts with marked proteinuria or renal failure
- Treatment
- Similar to ~IgAN
- Corticosteroids or cytotoxics for RPGN
- Prognosis
- Good (esp. in children), chronic renal failure & hypertension (rare)
