Henoch-Schönlein purpura

  • Clinical features
    • Young (<20 y/o), palpable purpura, abdominal pain, GI bleeding, arthropathy, fever, fatigue, ~IgA ↑ (50% p’ts), hematuria, proteinuria, macroscopic hematuria immediately after URI in children, NS (uncommon)
    • GN is very similar to ~IgA nephropathy
    • Skin: Leukocytoclastic vasculitis, dermal ~IgA deposits
  • Kidney biopsy is indicated only for p’ts with marked proteinuria or renal failure
  • Treatment
    • Similar to ~IgAN
    • Corticosteroids or cytotoxics for RPGN
  • Prognosis
    • Good (esp. in children), chronic renal failure & hypertension (rare)
