Incipient (microalbuminuria)

  • 5-10th year
  • Urinary albumin excretion (UAE)
    • 30-300 mg/day = 30-300 mg/g creatinine
  • Screen UAE at 5th yr for type 1 and at diagnosis for type 2 DM and yearly thereafter
  • Predicts CKD (ESRD within 10-20 years), CV dis. and mortality
  • False-positives: exercise, acute illness (e.g. infection, fever, CHF, marked hyperglycemia, severe hypertension)
  • Rx: ACEI/ARB/DRI
  • 30-45% progress to macroalbuminuria
